Vision, Aims and Objectives

 

Our Vision - ‘The population of mid Wales has access to safe, sustainable, bilingual and high quality integrated health and care services’

 

Aims Objectives
Aim 1: Wellbeing - Improve the wellbeing of the Mid Wales population
  •  Support the population of Mid Wales to become healthier with a focus on promoting healthy habits and behaviours in order to improve well-being
Aim 2: Enable people to live their best lives - Create a sustainable health and social care system for the population of Mid Wales which has greater focus on care in the right place.
  • Ensure that, where appropriate, people receive the majority of their care away from the acute hospital setting, which is in the right place as close to home as possible.
  • Improve the management of long term conditions within the community through better care within the community, promoting and supporting self-management.
  • Utilise technology to enable independent living, telemedicine and care.
  • Enable easy access to integrated health and care services for people who live in mid Wales.
  • Enable and supporting individuals to find meaningful occupation.

Aim 3: Rural Health and Care Workforce - Create a flexible and sustainable rural health and care workforce for the delivery of high quality health and care services.

  • Ensure the Mid Wales workforce is supported in continuous improvement, evidence based practice and research.
  • Develop and extend new/enhanced workforce roles, which will support integrated working across primary, community and secondary care. For example Physician Associates, Associate Nurses, Nurse Specialists, Consultant Nurses, Therapists and Social Care staff who support health services with medication for individuals.
  • Develop integrated community-based multidisciplinary teams.
Aim 4: Hospital Based Care and Treatment - Create an effective, efficient, sustainable and accessible Hospital Based Care and Treatment service for the population of Mid Wales with robust outreach services and clinical networks
  • Establish a clear strategy for Hospital Based Care and Treatment services for the population of Mid Wales which will include the establishment of clinical networks with patients treated in the right place.
  • Enable people to have appropriate accommodation options in Mid Wales, including Looked After Children, people with disabilities and older people. 
Aim 5: Communications, Involvement and Engagement - Ensure there is continuous and effective communication, involvement and engagement with the population of Mid Wales, staff and partners
  • Develop and implement clear mechanisms for engagement and consultation, and communication with the relevant population, and community/stakeholder groups; ensuring that plans, priorities for service development and evaluation of services are co-produced.
